Access Family Health Services is proud to announce that it has been recognized with six Community Health Quality Recognition (CHQR) badges from the U.S. Department of Health and Human Services' Health Resources and Services Administration (HRSA). The recognition comes as community health centers across the nation celebrate National Health Center Week 2026, highlighting the vital role health centers play in providing high quality, affordable health care. HRSA's CHQR program recognizes health centers that demonstrate excellence in clinical quality, patient outcomes, access to care, and health information technology based on the most recent Uniform Data System (UDS) reporting year.
Access Family Health Services has received three 2025 American Heart Association outpatient program achievement gold awards in recognition for its commitment to reducing the risk of heart disease and stroke by improving high cholesterol, Type 2 diabetes and blood pressure management. The awards recognize a commitment to following the latest evidence- and science-based care guidelines.
Access Family Health Services has been recognized by the American Heart Association and American Medical Association for its commitment to improving blood pressure (BP) control rates, earning Gold-level recognition as part of Target: BP™. The Gold award recognizes practices in which high blood pressure is controlled in 70% or more of the adult patients affected.
Access Family Health Services (ACCESS) is the recipient of six Community Health Center Quality Recognition (CHCQR) awards presented by the Health Resources and services Administration (HRSA).
